Appointments
Our office hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. Please call (248) 519-0305 during regular office hours to schedule an office appointment.
If you are unable to keep your scheduled appointment, we would appreciate a phone call 24 hours prior to that appointment, so we may schedule another patient in your place.
Prescription Refills
If you are in need of a prescription refill, please contact our staff during regular business hours, when the provider can review your child's record.
Please be ready to provide us with your child's name, date of birth, drug name, dosage, how often it is given, and pharmacy phone number. You will also be asked to provide a phone number to reach you if there are any questions. Refills will be phoned in as soon as possible, though please allow 24 hours for us to accommodate refill requests.
Things to Bring
In order for your child to be seen in our office, we ask that you bring an insurance card and referral form (if necessary). Please check with your insurance provider to determine if a referral is required.
In order for our providers to properly diagnose and treat your child, it is very important that you bring all medical records, x-rays (actual x-ray or ultrasound films) and laboratory results relating to your child's urological problem.
Unfortunately, our experience is that records mailed to us frequently do not arrive on time for the child's visit. We strongly recommend that you pick the records up and bring them to your visit.
Insurance Information
Our practice participates with many insurance companies. If you are a member of an HMO, or have an insurance that requires a referral, please make sure you bring a referral form for each visit, x-ray, laboratory study, or surgery. Please make referrals out to COMPREHENSIVE MEDICAL CENTER PLLC. Unfortunately, if you do not have a referral, you will have to reschedule your appointment. Comprehensive Medical Center PLLC is not responsible for obtaining your child's referral.
A co-payment required by your insurance is due at the time of service. If your insurance does not cover office visits, full payment is expected at the time services are rendered.
Please be aware of your insurance coverage and what is included with your policy. If you have any questions, please contact your insurance carrier prior to your visit.
In addition, please advise our receptionist of any changes in the patient's name, address, phone number, insurance coverage or referring physician that may have occurred since your last visit.
