Comprehensive Urology is committed to providing affordable, high quality care. We will file a claim with your insurance company using the exact information found on your insurance card. It is your responsibility to understand your plan’s specific rules regarding covered services, approved providers, referrals, authorizations and out-of pocket payments.

We accept Medicare, Medicaid and Tricare. All Comprehensive Urology providers are in-network with most major commercial insurances including: Aetna, Blue Cross Blue Shield, Cigna, Humana, United and many other carriers. If your insurance isn’t listed or you have questions, please contact the billing department at (248) 336-3188.
With HMO insurance policies (check your insurance card for the words HMO or call your carrier’s 800 number to inquire), you will need authorization from your primary care provider before seeing a urologist. Please bring your referral with you to your appointment. Your referral authorization must be received prior to your appointment to be seen by the physician.

Before you receive care, it is important to understand that, though much of your care may be covered by insurance, you are still responsible for out-of-pocket costs. Knowing what those costs may be and having a plan to pay will save you stress later.
It is important to have your correct insurance information on file to help us correctly bill your insurance. You will be responsible for any charges incurred if the information provided is not correct. Call your health plan to discuss what is covered and is not before your procedure or visit. There may also be certain procedures that require prior authorization or referral.
Private or cash paying patients without health insurance should be prepared to make payment of $150.00 at your appointment. If additional costs are incurred for more extensive testing or treatment, you will be required to pay in full at the end of your visit.

After insurance covers its portion of the bill, all remaining balances are your responsibility to pay.
When you receive your bill, you are expected to pay the full balance within 30 days. If you cannot pay, we offer financial assistance and payment plans. Your monthly statement shows all current activity on your account for the last 30 days. Previous  activity, such as payments or adjustments made beyond 30 days, will be on prior monthly statements.

  • Each monthly statement has multiple pages. Please review all pages for a breakdown of current account activity. 
  • Each date of service at Comprehensive Urology with current activity or a balance will be displayed on the statement. 
  • Activity displayed includes what is pending with insurance, payments from insurance, payments you have made in the last 30 days and any adjustments taken in the last 30 days. 
  • If previous balance is paid, it will not show on the current statement.

You may pay your bill online, by mail or in person. We accept all major credit cards: Master Card, Visa, American Express and Discover. You will need your Account number to access your account online.
